I authorize the Company to conduct this investigation for up to one year from the date below. Further, I agree to release, hold harmless, and waive any claims I may have against the Company and any and all schools, former employers, consumer reporting agencies and other persons or entities providing information to the Company (whether or not identified above) for any loss or injury I may sustain as a result of any disclosure related to this investigation.

  • I understand that if hired, I will be required to comply with all Company policies, including the Company’s Confidentiality Policy requiring, among other things, that any proprietary information to which I may be exposed to in the course of my employment be kept strictly confidential.
